Death Note replica questions - please answer?

I want to make my friend a death note notebook for Christmas because she's obsessed with Death Note, but I've barely seen the show and don't have enough time to familiarize myself with it before I have to make and give it to her. I need to know the basic structure of it.
I know the outside is a basic black notebook with the words "Death Note," but what does the inside look like? Is there anything really specific and crucial to the outside or inside that I need to know when making this notebook?

Side question, not as important, but what is the approximate size of the notebook, dimensions-wise?

Dimension of the notebook: about 22cm X 14cm OR 8 inch X 5.5inch

The outside should be pretty simple, Death Note written on the cover, the back is blank I think. Some other replicas look like the covers may be made of some kind of leather like material. It looks like it's the size of a normal notebook. The inside of the front cover has the rules of the death note written on it, as you can see here:

on the inside it looks like regular note book andgo to the libarary and take out the first one because at the beggining of every chapter there is a page saying "death note how to use it" the a bunch rules and the inside of the cover looks like that. I don't know the dimesions sorry.

Why don't you just buy one in an anime shop? It would save you the trouble of making it!
